Approved Organizations Must Submit CE Rosters Electronically to the AGD

In 2005 the AGD began requiring PACE-approved program providers to submit continuing education credits for all AGD members directly to the AGD. As of Jan. 1, 2017, providers were notified that they must use AGD’s online roster to submit CE hours earned by AGD members to the AGD. 

As of September 1, 2019 the AGD stopped accepting any continuing education (CE) verification forms or rosters through the mail, fax or email. All CE MUST be submitted electronically.  Any CE submitted through the mail, fax or email will be returned to the CE provider.

There are three options to submit AGD member attendance verification to the AGD:

  1. CSV file upload
    CSV stands for comma-separated values and is a simple file format used to store data, such as a spreadsheet or database. Files in the CSV format can be imported to and exported from programs that store data in tables, such as Microsoft Excel or Apache OpenOffice Calc. 

    You must be logged onto the AGD website under your provider ID Number.  Once logged in you can go to https://www.agd.org/continuing-education-events/pace/roster-csv. For more information please reference the AGD CSV Roster Submission Guide.

    To submit your file, set up your data in a common spread sheet, such as Microsoft Excel or use this template. The field/column headings must be in this order:
    1. AGD Member ID Number
    2. Course Date
    3. Course Title
    4. Number of Hours.
    5. AGD Subject Code
    6. Delivery Method
    7. Instructor Name
    8. Location.
  2. Online Roster Form
    Before accessing the AGD online roster form, make sure you have all the following information available.
    1. Contact person name and email
    2. Course title
    3. Name and credentials of the course instructor(s)
    4. Number of CE hours to be awarded
    5. Course type (lecture, participation or self-instruction) 
    6. AGD subject code representative of the topics taught
    7. Course location (city and state is sufficient) 
    8. Course completion date
    9. AGD Member ID numbers of all AGD members who attended the course.  Note: You will need to ask participants for their AGD Member ID number.  You do not need to submit any information to the AGD for participants who are not AGD members.

  3. API Process
    Organizations with IT professionals that are familiar with API processes should contact the AGD for a more streamlined method of submitting AGD member rosters to the AGD. For more information contact pace@agd.org.
