The Online Application for National PACE Approval Is Moving to a New Platform
In late 2018, AGD launched an online application for national PACE approval on the FluidReview™ application, which is powered by SurveyMonkey™. AGD is now planning to migrate to a new platform called SurveyMonkey Apply. We expect this platform to offer more flexibility and enhance an applicant’s experience.
As a result of this change, the application for national PACE approval will be unavailable to users from Friday, April 10, 2020, until Thursday, April 30, 2020. If you have not yet submitted an application for re-approval and your current approval ends between now and Oct. 1, 2020, we recommend that you submit your application no later than April 3, 2020, to ensure there is no lapse in approval.
If you are unable to submit an application by April 3, 2020, please submit by June 12, 2020. If your approval lapses but your application is later approved, you will be given retroactive approval back to the date of expiration. Email questions or concerns to PACE@agd.org.
